Big Beautiful Bill: Medicaid provider tax cuts, states lose $200B+ in matching funds

Federal · H.R.1, One Big Beautiful Bill Act, Pub. L. 119-21 · Signed Jul 4, 2025
Impact 9/10

The law freezes state provider taxes at current levels and, for Medicaid expansion states only, lowers the maximum allowable provider tax rate from 6 percent to 3.5 percent in half point annual steps from fiscal 2028 through 2032, with nursing facilities exempt. Provider taxes are a key mechanism states use to finance their share of Medicaid and draw down federal matching funds. CBO projects the provider tax provisions will cut gross federal Medicaid spending by about $226 billion over ten years, pressuring states to reduce benefits, eligibility, or provider payments. CMS issued implementing guidance in November 2025.
